Subject: Evidence request for [product/model] intended for [sales markets]

Hello [supplier contact],

We are reviewing the records for [exact product and variant]. Please confirm which of the following items apply before supplying documents. This is an intake request, not a conclusion that every listed document is legally required.

- Manufacturer legal identity and the source for contact details.
- Relevant economic operator information, if applicable to this supply chain.
- Product/model identifiers and batch or equivalent traceability records.
- Current label and packaging artwork or clear product photos, with version dates.
- Original warnings/instructions and available language versions.
- Relevant technical documentation, declarations or reports for the exact product, once applicability is established. Do not substitute unrelated certificates.

For each item, please identify the file name, date/version, product coverage and source. If an item is unavailable, say so explicitly. If you believe it is not applicable, include the reason for our reviewer to assess.

Please use [approved secure delivery channel] and respond by [agreed follow-up date]. Do not put private supplier documents on a public product page unless disclosure has been reviewed.

## Tracker instructions

Import supplier-evidence-tracker.csv into Excel or Google Sheets. Duplicate rows for each product. Keep applicability (unreviewed/applicable/not_applicable_with_reason) separate from evidence state (not_requested/requested/received_unverified/verified/needs_correction). A received file is not automatically verified. Add an owner, reviewer and next action; leave unknown information blank.
